Improved understanding of anemia in female athletes

When Kaitlyn Patterson's fatigue progressed to hyperventilating even during slow runs, and then forced her to quit high school distance running for the season, she knew something was very wrong.
Patterson had exercise-induced iron-deficiency anemia, a common, perplexing problem among elite female athletes, especially endurance runners. Later, as a University of Michigan sophomore, she was so interested in the topic that she applied for an undergraduate research position in the lab of Peter Bodary, U-M clinical assistant professor of movement science and health & fitness.
Patterson recently co-wrote a study challenging - and perhaps putting to rest - a popular hypothesis on what causes exercise-induced iron-deficiency anemia.
It's another in a succession of findings from separate research groups that debunk the notion that the iron-regulating hormone hepcidin causes exercise-induced anemia. Conventional science has suggested that rigorous exercise causes hepcidin spikes that result in anemia.
"The U-M finding is significant because we want to provide physicians with the most accurate information regarding exercise-induced anemia," Bodary said.
The downside is that hepcidin isn't the anemia cure-all the scientific community had hoped.
Bodary said that physicians already do a nice job of helping athletes recover from exercise-induced anemia by providing iron supplementation and educating athletes about optimizing iron absorption. The challenge now is understanding what causes anemia in order to prevent it.
The U-M study is the first known to compare hepcidin levels in competitive college female runners against those of non-exercisers, Bodary said. About half the runners in the study had a history of anemia and about 85 percent supplemented with iron.
Researchers wanted to see if hepcidin remained high in resting athletes, which could link elevated hepcidin with exercise-induced iron-deficiency anemia. However, overall hepcidin levels in both groups were about equal, Bodary said.
Will scientists have to return to the starting line? Not necessarily. A physician familiar with Bodary's research contacted him about a female patient with long-term anemia that is hypothesized to have elevated levels of resting hepcidin. Although she didn't exercise regularly, her blood work resembled that of an athlete with the disease, Bodary said.
By analyzing the woman's blood samples, Bodary's lab hopes to learn if high hepcidin drives her anemia. Results could help scientists decode what causes anemia in both the general population of women and in elite athletes.
Meanwhile, Patterson remains healthy since her first struggles with anemia.
"I still run, and have returned to cross country skiing and have taken up cycling," she said. "I'm planning on ski racing this winter and putting together a solid triathlon season. I've been fortunate in that I responded well to iron supplementation, so I've stayed healthy since my original bout with anemia."

Too much sport 'may be bad for teens' health'

Teenagers have long been told that being active and taking part in sports is good for their health. But new research suggests that too much sport for teenagers could negatively impact their well-being just as much as too little sport.
The US Department of Health and Human Services recommends that young people aged between 6 and 17 years carry out at least 60 minutes of physical activity a day, equating to 7 hours a week.
But researchers from Switzerland and Canada say their study, published in the BMJ journalArchives of Disease in Childhood, suggests that 14 hours of physical activity a week is best for promoting good health in teenagers. However, they found that more than 14 hours appears to be detrimental to their health.
To reach their findings, the investigators surveyed more than 1,245 teenagers aged between 16 and 20 from Switzerland.
All participants were required to answer questions regarding demographics, height and weight, socioeconomic status, sports practice, sports injuries and well-being.
Their well-being was assessed using the World Health Organization (WHO) Well-Being Index, which provides scores between 0 and 25. A score below 13 is an indicator of poor well-being.
Of the participants, 50.4% were male with a mean age of 17.95 years. Almost 9% of these males were overweight or obese. The overall average well-being score for all participants was 17.
The researchers categorized sports participation as low (0-3.5 hours a week), average (3.6-10.5 hours), high (10.6-17.5 hours), and very high (more than 17.5 hours).
Low sports activity was found in 35% of subjects, 41.5% had average activity, 18.5% had high, while 5% had very high.

Very high activity 'just as bad' as low activity

The investigators found that participants in the low and very high activity groups were more than twice as likely to have well-being scores below 13, compared with subjects in the average group.
The researchers say this corresponds to an "inverted U shaped" link between weekly duration of sports practice and well-being.
They also found that the highest well-being scores were obtained by participants who carried out around 14 hours of physical activity a week, but beyond 14 hours resulted in lower well-being scores.
Commenting on their findings, the researchers say:
"Physical activity has been associated with positive emotional well-being, reduced depressive, anxiety and stress disorders, and improved self-esteem and cognitive functioning in children and adolescents.
We found that sports practice apparently ceased to be a protective factor and became an independent risk factor for poor well-being when practicing more than twice the 7 recommended hours per week."
The investigators note that their study highlights the importance for physicians caring for adolescents to monitor their level of sports practice and ask them about their well-being.
"Regardless of their decision to pursue their level of practice, these adolescents probably need a supportive and closer follow-up of their health and well-being. Our findings can inform guideline panels who produce recommendations on sports practice for adolescents," they add.

Students may need a break from school after concussion

A new clinical report from the American Academy of Pediatrics suggests that student athletes who have suffered concussion following head injury may need to take a break from academic learning.
In a report released at the American Academy of Pediatrics (AAP) National Conference and Exhibition in Orlando, FL, the authors say that children and adolescents with concussion may need more time to adjust to learning following injury, and that returning to academics could "worsen" concussion symptoms.
The AAP say they based their report on expert opinion and an adaption of a concussion management program developed at the Center for Concussion at the Rocky Mountain Hospital for Children in Denver, CO.

Symptoms of concussion

Concussion is a form of traumatic brain injury that can be caused by a blow to the head or a fall that jolts the head and brain.
According to the Centers for Disease Control and Prevention (CDC), concussion can occur in any sport or recreation activity, therefore all parents, coaches and athletes should learn signs and symptoms of the injury.
Some common symptoms as stated by the CDC are:
  • Headache or "pressure" in the head
  • Nausea or vomiting
  • Double or blurry vision
  • Sensitivity to noise and/or light
  • Concentration or memory problems.

AAP recommendations following concussion

The AAP says that student athletes who have concussion may need to take some time out from academic learning, or symptoms could worsen.
The authors say that previous research has shown that it usually takes up to 3 weeks for a school-aged student to recover from concussion.
A 2010 AAP clinical report suggests that students may need to stay home from school for a period of "cognitive rest" if symptoms are severe, and those with "mild or tolerable" symptoms who return to school should have some "adjustments" made to assist their learning.
These may include shortening of the school day, reduction of workload, or allowance of more time to complete assignments or take tests.
The authors add that students with symptoms that last longer than 3 weeks may need more "formalized academic accommodations."
"Students appear physically normal after a concussion, so it may be difficult for teachers and administrators to understand the extent of the child's injuries and recognize the potential need for academic adjustments," says Dr. Mark Halstead, a lead author of the report.
"But we know that children who've had a concussion may have trouble learning new material and remembering what they've learned, and returning to academics may worsen concussion symptoms."

Each child 'requires individualized approach'

With these factors in mind, the AAP recommends that a team consisting of the students' pediatrician, family members and individuals related to the children's academic timetable and physical fitness should work together to ensure the best approach for their return to learning following concussion.
Dr. Halstead explains:
"Every concussion is unique and symptoms will vary from student to student, so managing a student's return to the classroom will require an individualized approach.
The goal is to minimize disruptions to the student's life and return the student to school as soon as possible, and as symptoms improve, to increase the student's social, mental and physical activities."
The AAP notes that further research is warranted regarding the role and effects of cognitive rest following concussion, and to better understand the best ways to help a student who is recovering from concussion.

Exercise can boost teens' academic performance

Many teenagers are more interested in watching TV and playing video games than exercising. But new research has presented them with another reason to get active - regular moderate to vigorous exercise could boost their academic performance.
According to the Centers for Disease Control and Prevention (CDC), regular physical activity in childhood and adolescence has many significant benefits, including helping to build healthy bones and muscles, improving strength and endurance and increasing self-esteem.
But statistics from the CDC show that in 2011, only 29% of high school students participated in 60 minutes of physical activity a day - the amount of activity recommended by the US Department of Health and Human Services.
Researchers from the UK say that if their findings - published in the British Journal of Sports Medicine - are confirmed through further research, it could present significant implications for public health and education policy.
The research team analyzed a sample of 5,000 children who were a part of a Children of the 90s study.
The children, aged 11, were required to wear an accelerometer on an elasticated belt for a period of 3-7 days, in order for the researchers to monitor their daily duration and intensity of physical activity.

Implications for public health policy

Results from the accelerometer showed that on average, boys carried out 29 minutes of moderate to vigorous exercise each day, while girls carried out 18 minutes. The researchers note that this is significantly less than the 60 minutes of exercise each day recommended by health officials.
These results were then compared with the children's academic performance in English, mathematics and science in the compulsory national test key stage 1 at age 11, key stage 2 at age 13, and General Certificate of Secondary Education (GCSE) at ages 15/16.
The findings revealed that at age 11, higher levels of moderate to vigorous exercise correlated with better academic performance across all three subjects for both boys and girls.
Girls in particular demonstrated a significant improvement in science performance as a result of physical activity.
At age 13, better academic performance was also linked to increased physical activity.
At age 15/16, every additional 17 minutes of exercise a day for boys and 12 minutes for girls was linked to better examination results. Again, females demonstrated the highest benefit of exercise through their science results.
The researchers say that these results suggest that devoting more time to physical education benefits not only the health and well-being of teenagers, but also their academic attainment, encouraging the need for public health and education policy interventions.
They add:
"If MVPA (moderate-vigorous physical activity) does influence academic attainment, this has implications for public health and education policy by providing schools and parents with a potentially important 'stake' in meaningful and sustained increases in physical activity."
Furthermore, they note that although it could be a "chance finding" that increase in physical activity was linked to better science performance in females, this could also signify gender differences in the brain as a result of exercise.
However, they add that further research is required in order to confirm and understand this finding.

A daily bout of exercise can counter the harmful effects that short-term inactivity and overeating have on health, according to a new study published in The Journal of Physiology.
Evidence already exists that even a few days of consuming more calories than you burn can be harmful to health.
The new study, from the University of Bath in the UK, takes a step further and suggests a daily dose of exercise can bring health benefits that go beyond just helping to burn off excess calories.
Speaking about their work, co-author Dr. James Betts says:
"This new research shows that the picture is more sophisticated than 'energy' alone: exercise has positive effects even when we are actively storing energy and gaining weight."
He and his colleagues invited 26 healthy, normally active young male volunteers, aged from late teens to early 30s, to consume more calories than normal for a week.

Researchers found that daily exercise counteracts negative health effects of short-term inactivity and overeating.
Half of the volunteers were asked to restrict their physical activity (to below 4,000 steps per day), in order to generate an energy surplus, and the other half were asked to run for 45 minutes per day on a treadmill (at 70% of maximum oxygen intake).
The inactive group was asked to consume around 50% more calories than normal, while the exercise group was asked to consume 75% more, so that everyone's net daily energy surplus was the same.
At the start and the end of the week, the volunteers gave fasting blood samples, underwent oral glucose tests and also had small samples taken of abdominal fat.

Unhealthy declines in blood sugar control

After just 1 week, the differences between the two groups were striking.
The inactive group showed a significant, unhealthy decline in blood sugar control, and their fat cells were over-expressing genes linked to disrupted nutritional balance and unhealthy metabolism, and under-expressing genes linked to healthy metabolism.
However, these effects were markedly less in the exercise group: their blood sugar levels remained stable, and while their fat cells did show some changes in gene expression, these were significantly less "undesirable."
The researchers conclude:
"Vigorous-intensity exercise counteracted most of the effects of short-term overfeeding and under-activity at the whole-body level and in adipose tissue, even in the face of a standardized energy surplus."

Despite consuming more calories, exercise group was still better off

Senior author Dr. Dylan Thompson says:
"A critical feature of our experiment is that we matched the energy surplus between groups - so the exercise group consumed even more energy and were still better off at the end of the week."
He suggests the message from this study for those entering a period of overconsumption and inactivity, such as that faced by Christmas revellers, is that "a daily bout of exercise will prevent many of the negative changes from taking place even though you are gaining weight."
